Liability Waiver & Assumption of Risk

[[LEGAL REVIEW PENDING]] — this page is a plain-language starting point, not a finished legal waiver. The actual liability waiver and assumption-of-risk agreement must be drafted (or reviewed) by an attorney and signed by every participating family before an athlete's first session — this page does not itself collect a signature.

Assumption of risk

Athletic training carries an inherent risk of injury, including but not limited to sprains, strains, collisions, and more serious injury. By participating in Receiver Takeover programs, athletes and their parents or guardians acknowledge and accept these risks. [[FINAL WAIVER LANGUAGE PENDING LEGAL REVIEW]]

Who must sign

[[Confirm: signed by a parent or legal guardian on behalf of every athlete under 18, before that athlete's first session — season package, drop-in, or 1-on-1 alike?]]

Medical information

[[Confirm what, if any, medical information (allergies, conditions, emergency contact) is collected as part of the signed waiver, and how it is stored and protected. This site does not currently collect medical or date-of-birth information anywhere — see the Privacy Policy.]]

How it's signed

[[Confirm the actual signing process — paper form at the first session, a separate e-signature tool, or something else — once decided.]]
